General Consent and Liability Waiver:
In consideration of my exercise, nutrition & coaching program, I expressly agree and contract, on behalf of myself, my heirs, executors, administrators, successors and assigns, that The Fitness Project, Inc., shall not be liable for any damages arising from personal injuries or psychological damages (including death) sustained by me, or my guest in, on, or about the premises, or as a result of the use of the equipment or facilities, regardless of whether such injuries result, in whole or in part, from the negligence of The Fitness Project, Inc.
By the execution of this agreement, I accept and assume full responsibility for any and all injuries, damages (both economic and non-economic), and losses of any type, which may occur to me, and I hereby fully and forever release and discharge The Fitness Project, Inc. and all coaches from any and all claims, demands, damages, rights of action, or causes of action, present or future, whether the same be known or unknown, anticipated, or unanticipated, resulting from or arising out the use of said equipment and facilities.
I expressly agree to indemnify and hold The Fitness Project, Inc. harmless against any and all claims, demands, damages, rights of action, or causes of action, of any person or entity, that may arise from injuries or damages sustained by me due to any exercise, diet & coaching programs.
I agree to be solely responsible for the safety and well-being of myself, both mentally and/or physically. I understand that The Fitness Project, Inc. is not accountable for my actions and choices pertaining to exercise and diet.
I understand and acknowledge that exercise and dieting involve risk of serious injury, including permanent disability and death & that I should consult with a doctor before starting any new fitness & diet routine.
I understand and acknowledge that The Fitness Project, Inc. coaches are not certified dieticians and therefore any information given to me on nutrition or diets is not a recommendation or diet solution, but rather a guide to follow based on my own request for help and or guidance pertaining to my fitness goal. I also understand that I am in control of my own body and know it best therefore if at any time I feel discomfort, pain or overall fatigue, I am to remove myself from the fitness training and inform my trainer of such conditions as well as consult with a doctor. I also recognize and agree that all cash or tender exchanges between The Fitness Project, Inc. and I are non-refundable. I agree that under no circumstances can I take legal action against The Fitness Project, Inc. if I am unsatisfied with my results. I also recognize that I am signing this waiver under no pressure & under my own will to do so.
NOTE: Results may vary.
The Fitness Project app stores all your information, including progress, workouts, and diet information. Therefore, this app is a resource that is strongly suggested by The Fitness Project Team (to be downloaded from Apple or Google Play). There is a small monthly fee ($35/month) for app usage. This fee will be waived for the duration of your 6 weeks program and will start as an automatic monthly charge thereafter. This information can still be accessed/utilized after your 6 weeks program for the same monthly fee.
